ST. SEBASTIAN'S CYO BASKETBALLREGISTRATION FOR CYO BASKETBALL LEAGUE 2005-2006 GRADES 3-8
The following forms and documents must be completed and submitted:
1. Registration Form 2. CYO Parent Code of Conduct 3. Copy of Birth Certificate All new players must submit a copy of a birth certificate. Returning players do not need to submit a copy of a birth certificate. 4. A check for $80.00 made payable to "CYO"

The deadline for registrations is September 26, 2005. There is no guarantee that a player will be placed on a team if the registration forms are received after September 26.
Player evaluations will be conducted on October 1-2 at Twin Hills School. Player evaluations are mandatory for 4th graders-8th graders and each player must attend one of the two sessions. There are no player evaluations for 3rd graders. Returning players will be given first priority for placement on teams.
Practices begin the week of October 17. Practice schedules will be available approximately October 14. While we try to schedule practices for a consistent date and time each week, it is often not possible since schools do not release gym time to us in a consistent manner.
The season consists of a maximum of 10 games played on either a Saturday or Sunday beginning on December 3-4. There are no games on December 24-25 and December 31-January 1. The season ends on February 18-19. Playoffs for grades 4-8 are scheduled for February 25-26 and March 4-5 (single elimination). Game schedules will not be available until approximately November 20.
